In-Home Nursing Care
Whether you need short-term recovery or long-term support, our in-home nursing care provides reliable and professional assistance. We’re here to help.
Our In-Home Nursing Care Services
Since 2004, LifeWorx has served over 2,500 families with compassionate and reliable in-home nursing care throughout New York, Connecticut, New Jersey, and Florida. Our in-home nursing care provides advanced medical care for individuals with complex health needs, allowing them to receive high-quality treatment in the comfort of their home. Our nursing team is led by our highly qualified Director of Patient Services, Dwiann Morgan, who is licensed and oversees the Directors of Nursing in each state where we operate.
Unlike traditional nursing homes, where patients often experience rotating staff and limited personalized attention, in-home nursing care ensures our clients receive consistent, customized support based on their medical history and preferences. Our nurses also work closely with physicians and other healthcare providers to create a seamless care experience, so you can rest assured knowing they’re in the best hands.

Post-Operative In-Home Nursing Care
Recovering from surgery requires careful attention to ensure proper healing and minimize the risk of complications. Without proper attention, patients may face complications such as infections, blood clots, or mobility issues.
Our in-home nursing services provide essential post-operative support, including wound care, pain management, mobility assistance, and complication monitoring, all within the client’s home. This not only ensures a comfortable recovery but also lowers the risk of readmissions. Unlike nursing homes, which can be overwhelming and impersonal, in-home nursing care prioritizes the patient’s needs while maintaining their autonomy.
Medication Management & Administration
Our in-home nursing care services ensure that clients take the correct medications at the right times and dosages. Unlike nursing homes, where patients may not receive individualized attention, home nursing care provides one-on-one medication oversight, improving safety and effectiveness.
Our nurses can coordinate with healthcare providers to assess the medication effectiveness, adjusting dosages or schedules accordingly. Additionally, for clients with cognitive impairments or mobility challenges, our nurses ensure timely administration and can educate both the client and their family about the purpose and side effects of each medication.
Take the first step toward better care

Patient Advocacy
Our in-home nurses serve as strong advocates for their clients, making sure their needs are prioritized and that they receive the highest standard of care. A nurse can act as a liaison, ensuring that the client’s questions and concerns are addressed during appointments or hospital follow-ups.
Our in-home nurses can also help families understand medical terminology, empowering them to make informed decisions about care plans and treatments. In addition to coordinating care, our nurses can identify potential issues and bring them to the attention of physicians for better health outcomes and communicate to family members who may not be able to be physically present.
Chronic Condition Monitoring
For clients with chronic conditions, such as diabetes, heart disease, or COPD, in home nursing care provides constant monitoring and management to ensure stability and prevent complications. Unlike nursing homes, which often follow a generalized care model, in-home nursing care offers specialized monitoring, improving long-term health outcomes and quality of life.
Our nurses work with physicians to adjust care plans as needed. They also focus on reducing hospital readmissions by identifying early warning signs. With their attentive care, clients can better manage their chronic conditions, maintain a higher quality of life, and avoid unnecessary medical emergencies.

How it Works

Discuss your in-home nursing care needs
Documenting medical needs is the easy part. Where the work comes in is understanding your loved one’s personality, their triggers and limitations, any barriers to potentially accepting care for the first time, and the little things we can add to their lives by way of a caregiver that will put a smile on their face.
Meet our in-home care nurses
You’ll meet several fully vetted nurses, each with a summary of their skills and experience, activities they enjoy engaging in with clients, personality traits, background and interests, quotes from their reference transcripts, and a photo from their in-person interview. From there, you can interview either in-person or over the phone or video call or simply decide based on their profile.
Services begin
We do our best to keep things simple and find the perfect match for your loved one on our first time around and are encouraged by how often this happens. However, if things are not quite clicking, our Client Managers will take your feedback and go back to the drawing board for a more fitting solution. Above and beyond is the expectation, not the exception, with peace of mind as the ultimate outcome.

Our In-Home Care Nurses
In certain situations, extended supervision and care by an in-home nurse is essential. In-home nurses bring a higher level of medical expertise, making them uniquely qualified to handle complex cases.
With advanced training in patient care, they can manage medical devices like catheters, feeding tubes, or ventilators, and respond quickly to emergencies. For example, a nurse working with a cardiac patient might recognize the early signs of heart failure and take immediate action.
Regardless of the circumstance, we will work with you and their doctors to ensure that your loved one is properly cared for. LifeWorx has in-home nurses available in New York, New Jersey, Connecticut, and Florida. Many are even licensed in multiple states. And all are professionals in clinical care and ready to attend to a client’s every need, while bringing a warm, empathetic presence into your loved one’s home.

The LifeWorx Difference
Our mission is to bring comfort, independence, and joy back into your loved one’s life. Our commitment to quality begins with our CHAP accreditation and state licensing, ensuring we meet the highest industry standards for safety and care.
Unlike corporate or franchised agencies, LifeWorx is privately owned and operated, which grants us the opportunity to work closely with clients and their families to address their unique circumstances with a customized plan. Our rates are straightforward and all-inclusive, with no hidden fees or overtime charges.
Anyone can walk into our ground level offices and speak with our team directly. Our in-office team works together to ensure every decision is focused on delivering the best care for your loved one.
Institutional settings tend to have strict routines, but our services are flexible and designed to fit seamlessly into your loved one’s day-to-day life. Whether they need a companion for a morning walk or a nurse to manage post-surgical care, we can adapt to their preferences and needs.
We understand the importance of trust and consistency in home care. Unlike other agencies that shuffle caregivers frequently, we prioritize stability by minimizing nurse changes and acting only on feedback from you and your family. This means your loved one benefits from a familiar face and a consistent level of care, making every day just a little bit easier.
